is a nursing and home care

agency with a staff of

trained and caring personnel to

 provide you with all of your

 nursing, personal, staffing

and home care needs in the

home, hospital or nursing home.

We have been serving the:

~ Quality Care Assurance

 All CHP staff are licensed or certified

by the state as well as carefully screened

and bonded for your safety. We have a registered nurse and a staff coordinator

on call 24 hours each and every day.

~ Credentials

 Carolina Health Professionals is

licensed by the NC Department of

Health and Human Services- Division

of Facility Services and a member of

the National Hospice and Palliative

Care Association.

~ Services

  Our care and service is individually

 tailored to meet the needs of clients recovering from illness, injury or surgery

as well as clients with long-term

disabilities at home, in the hospital or

nursing home. Short-term or long-term

care can be provided. Care and services

may be provided through visits by a nurse

or nursing assistant/sitter or hourly

services, depending on your care needs.

We provide care from 4 hours to 24 hours

each day, once a month or every day.
 

~ Specialized Program

 We participate in many home care

programs such as CAP-DA, CAP-MR/DD,

CAP-C, Veteran’s Administration

Community Services, PCS, etc.
